Volunteer with Oak Lodge Sanitary District
Oak Lodge Sanitary District has a number of standing committees as well as specially appointed committees for specific purposes. The District routinely asks for volunteers to fill one or more vacancies that regularly arise on one or more of the committees. Appointees must reside in the District. Low Income Rate Relief Program
Effective July 1, 2005, Oak Lodge Sanitary District instituted a Low Income Utility Rate Relief Program. This program allows eligible customers to apply for a discounted rate for their sanitary sewer bill. Approved applicants will receive a fifty-percent reduction in both the base and consumption charges for sanitary sewer service. Surface water management program charges will remain unchanged at $6.00 per month. Please read the program information carefully and complete the application to Oak Lodge Sanitary District as noted on the form.
Auto Pay Authorization Form
Effective January 1, 2007, Oak Lodge Sanitary District offers customers the convenience of automatic debit. With this service, the District will automatically debit your bank, savings and loan, or credit union account for the amount of your service charges. If you would like to request this service, please read the program information carefully and fill out the application at the bottom of the form. Be sure to include a voided check with the completed application.

Request for Change of Billing Status
Customers who wish to request a change of billing status may fill out this form and return it to the District's Administration Office. Remember to provide a complete explanation for the status change requested, and sign and date the bottom of the form. Also, please note that the District will not adjust an account's billing status for temporary vacancies.
